Pragmatic Free Trial Meta

Pragmatic Free Trial Meta is a non-commercial, open data platform and infrastructure that supports research on pragmatic trials. It shares clean trial data and ratings using PRECIS-2 which allows for multiple and varied meta-epidemiological studies that compare treatment effects estimates across trials that have different levels of pragmatism and other design features.

Background

Pragmatic studies provide real-world evidence that can be used to make clinical decisions. However, the use of the term "pragmatic" is not consistent and its definition and evaluation requires clarification. Pragmatic trials must be designed to guide clinical practice and policy decisions, rather than to prove an hypothesis that is based on a clinical or physiological basis. A pragmatic trial should also try to be as similar to actual clinical practice as is possible, including its participation of participants, setting up and design as well as the implementation of the intervention, and the determination and analysis of outcomes as well as primary analysis. This is a major distinction between explanatory trials as described by Schwartz and Lellouch1, which are designed to prove the hypothesis in a more thorough manner.

The trials that are truly practical should be careful not to blind patients or clinicians as this could lead to distortions in estimates of the effect of treatment. Practical trials should also aim to enroll patients from a variety of health care settings so that their results can be compared to the real world.

Finally, pragmatic trials should focus on outcomes that are vital for patients, such as quality of life or functional recovery. This is particularly relevant for trials involving the use of invasive procedures or potentially dangerous adverse events. The CRASH trial29, for instance was focused on functional outcomes to compare a 2-page case-report with an electronic system for monitoring of patients admitted to hospitals with chronic heart failure. In addition, the catheter trial28 focused on symptomatic catheter-associated urinary tract infections as the primary outcome.

In addition to these features, pragmatic trials should minimize the procedures for conducting trials and requirements for data collection to reduce costs and time commitments. Finaly, pragmatic trials should aim to make their findings as relevant to actual clinical practices as they can. This can be achieved by ensuring that their primary analysis is based on the intention to treat approach (as defined in CONSORT extensions).

Despite these guidelines, many RCTs with features that challenge pragmatism have been incorrectly self-labeled pragmatic and published in journals of all types. This can lead to false claims of pragmatism and the usage of the term should be standardized. The creation of the PRECIS-2 tool, which offers an objective standard for assessing practical features is a great first step.

Methods

In a pragmatic research study it is the intention to inform clinical or policy decisions by demonstrating how an intervention could be integrated into routine treatment in real-world situations. Explanatory trials test hypotheses concerning the cause-effect relationship within idealised conditions. In this way, pragmatic trials can have lower internal validity than explanation studies and be more prone to biases in their design as well as analysis and conduct. Despite these limitations, pragmatic trials can provide valuable information to decision-making in the context of healthcare.


The PRECIS-2 tool evaluates an RCT on 9 domains, with scores ranging between 1 and 5 (very pragmatist). In this study the areas of recruitment, organization as well as flexibility in delivery flexible adherence, and follow-up received high scores. However, the principal outcome and the method for missing data scored below the pragmatic limit. This suggests that it is possible to design a trial with high-quality pragmatic features, without compromising the quality of its results.

A typical feature of pragmatic research is that researchers try to make their findings more relevant by studying subgroups within the trial. This can result in unbalanced analyses with less statistical power. This increases the risk of missing or misdetecting differences in the primary outcomes. This was the case in the meta-analysis of pragmatic trials due to the fact that secondary outcomes were not adjusted for covariates' differences at the time of baseline.

Additionally the pragmatic trials may be a challenge in the gathering and interpretation of safety data. This is due to the fact that adverse events are usually self-reported and are susceptible to reporting errors, delays, or coding variations. It is crucial to increase the accuracy and quality of the outcomes in these trials.

Many studies have attempted classify pragmatic trials using a variety of definitions and scoring methods. Schwartz and Lellouch1 created a framework for distinguishing between research studies that prove a physiological or clinical hypothesis, and pragmatic trials that help in the choice of appropriate therapies in the real-world clinical setting. The framework was composed of nine domains assessed on a scale of 1-5, with 1 being more informative and 5 being more pragmatic. The domains covered recruitment and setting up, the delivery of intervention, flex adherence and primary analysis.

The original PRECIS tool3 was an adapted version of the PRECIS tool3 that was based on the same scale and domains. Koppenaal et al10 developed an adaptation of this assessment, called the Pragmascope that was simpler to use for systematic reviews. They discovered that pragmatic systematic reviews had a higher average scores across all domains but lower scores in the primary analysis domain.

This difference in primary analysis domain can be due to the way in which most pragmatic trials approach data. ???? ????? , however don't. The overall score was lower for pragmatic systematic reviews when the domains of the organization, flexibility of delivery and follow-up were combined.

It is important to understand that a pragmatic trial does not necessarily mean a low-quality trial, and in fact there is an increasing number of clinical trials (as defined by MEDLINE search, however it is neither specific or sensitive) which use the word "pragmatic" in their abstract or title. These terms may signal an increased appreciation of pragmatism in titles and abstracts, but it's unclear if this is reflected in content.

Conclusions

In recent times, pragmatic trials are gaining popularity in research as the value of real world evidence is becoming increasingly acknowledged. They are randomized clinical trials that compare real-world care alternatives rather than experimental treatments under development. They have patients which are more closely resembling the ones who are treated in routine medical care, they utilize comparators which exist in routine practice (e.g. existing medications), and they rely on participant self-report of outcomes. This method is able to overcome the limitations of observational research, like the biases associated with the reliance on volunteers, as well as the insufficient availability and coding variations in national registries.

Other advantages of pragmatic trials are the ability to utilize existing data sources, and a greater probability of detecting significant changes than traditional trials. However, these trials could be prone to limitations that compromise their credibility and generalizability. For instance, participation rates in some trials could be lower than anticipated due to the healthy-volunteer effect and financial incentives or competition for participants from other research studies (e.g. industry trials). Practical trials are often restricted by the need to enroll participants quickly. Practical trials aren't always equipped with controls to ensure that any observed differences aren't caused by biases in the trial.

The authors of the Pragmatic Free Trial Meta identified 48 RCTs that self-described themselves as pragmatic and were published until 2022. They evaluated pragmatism using the PRECIS-2 tool, which consists of the eligibility criteria for domains and recruitment criteria, as well as flexibility in adherence to interventions, and follow-up. They found that 14 of these trials scored as highly or pragmatic pragmatic (i.e. scores of 5 or more) in one or more of these domains, and that the majority of these were single-center.

Studies with high pragmatism scores tend to have broader criteria for eligibility than conventional RCTs. They also contain populations from various hospitals. The authors argue that these characteristics can help make pragmatic trials more effective and applicable to daily practice, but they do not guarantee that a trial using a pragmatic approach is completely free of bias. The pragmatism principle is not a fixed characteristic and a test that doesn't have all the characteristics of an explanation study could still yield valid and useful outcomes.
